Hazardous Waste Transport Licence Application Package

New applicants can contact the Environmental Management Branch in Victoria, British Columbia in person, by phone, email, fax or in writing to request an application package for a transport licence. Alternately, applicants can print all application package material, with the exception of the Hazardous Waste Legislation Guide, from this web site. Note that the legislation guide is required in order to prepare for the transport licence test.

The application package will include the following:

  1. a "Have You Enclosed the Following?" form (PDF/38 KB) UPDATED: AUGUST 2009
  2. a Product List and Order form
  3. an Application for a Licence to Transport Hazardous Waste, along with Instructions to Transport Licence Applicants (PDF/302 KB)
  4. an Additional Information form (PDF/18 KB)
  5. a Covering Letter (PDF/66 KB)
  6. a copy of Hazardous Waste Transport Licence Test Instructions (PDF/44 KB)
  7. a copy of Instructions Regarding Financial Security (PDF/51 KB)
  8. the recommended format for an Irrevocable Letter of Credit/Guarantee (PDF/25 KB)
  9. the recommended format for a Surety Bond (PDF/44 KB)
  10. Sample Safekeeping Agreement (Word/151 KB), along with Instructions for Completion (PDF/17 KB) New: October 2009
  11. a sample of a Consignee Confirmation Letter (PDF/20 KB)
  12. an Acknowledgement That No Interest Paid on Cash Deposit form (PDF/22 KB)
  13. a sample of a general Contingency Plan (PDF/31 KB)
  14. a Contingency Plan for the Spill of Lead Acid Batteries (PDF/50 KB)
  15. a template for companies to create their own Spill Reporting Information form (Word/46 KB)
