Appendix II. Examples of Forms Used in Landscape IPM

These reports are filled in by city landscape staff who submit the information to the city IPM Coordinator. Both forms ask for detailed information, using a simple check-off format as much as possible to make sure the facts needed to make decisions are available.

C. Sticky Card Counts

This form was designed for use in monitoring pests in bedding plant greenhouses, however, it could also be adapted for use in other ornamental greenhouses.

Each box on the form is used to record information from one yellow sticky card at four monitoring visits. Record the greenhouse, card number, card location (bench, floor, or ceiling), and make a note if the card is changed. If it is not, circle the counted insects so they will not be included at the next monitoring visit. Record the number of insects on the card in the appropriate column; WF = whitefly; FG = fungus gnat; SF = shorefly. To avoid confusion, write "0" rather than leaving a blank when no insects are present.
